Building form for requesting services and I need to populate rows of form fields based on the number of services requested. I have attempted to do so using Javascript, but when displayed the form distortd the entire page.

There are a few different ways of doing this. Depending on your comfort level with Javascript, jQuery (http://jquery.com) may be your best bet. It provides very easy function calls for manipulating the DOM client-side.

Either that, or table manipulation functions to add cells / rows. The problem is, there's lots of code that needs to be implemented and there's always browser issues.
