I do this today with a combo of ListX and PageBlaster 3.0.

I use ListX to create a span id=NewTitle with the title I want for the page.

Then I create a PageBlaster rule that replaces the title with the contents of the NewTitle span.

I do this for Titles, Descriptions and Keywords.

This way A single Page that contains the data about 100 Adult Family homes selected by a Query string can have unique metatags for each home.
